1.0 Preamble
In compliance with the provisions of the Public Procurement Act 2007, the Federal University of Health Sciences, Otukpo, seeks quotations from competent and qualified contractors, suppliers, and service providers to quote for the underlisted projects to be funded under the specified interventions.

3.0 Scope of Work / Description of Projects
The scope of procurement of goods includes procurement, installation, and training on the use of Teaching and Research Equipment and Furniture.
4.0 Eligibility Requirements
Prospective Applicants should submit the following documents:
- Evidence of a Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC~1~ or CAC~2~ and CAC~7.
- Evidence of the Company's Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2026.
- Evidence of current Pension Compliance Certificate valid till 31st December 2026 (applicable only to bidders with 15 or more staff).
- Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till December, 2026 (applicable only to bidders with 5 or more staff).
- Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2026.
- Evidence of registration on the National Database of Federal Contractors by submission of an interim Registration Report (IRR) expiring on June 30, 2026 or a valid certification issued by BPP.
- Evidence of registration with the Original Equipment Manufacturer (OEM).
- Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal University of Health Sciences or the Bureau of Public Procurement is a former or present Director, shareholder, or has any pecuniary interest in the bidder and confirming that all information presented in its bid is true and correct in all particulars; that no director has been convicted in any country for any criminal offence relating to fraud or any financial impropriety; that the company is not in receivership, insolvency, or winding-up proceedings; and that the company does not have any existing loan or financial liability classified as doubtful, nonperforming, bad, or outstanding for the last four (4) months.
- Company's Audited Accounts for the last three (3) years: 2023, 2024, and 2025.
- Reference letter from a reputable commercial bank in Nigeria indicating willingness to provide a credit facility for the execution of the project when needed.
- FOR PROCUREMENT OF AMBULANCE (LOT 2): Bidders MUST present a letter of Authorization from accredited vehicle Manufacturers approved by National Automotive Design and Development Council (NADDC).
5.0 Collection of Documents
The Standard Bidding documents for goods can be accessed upon payment of a non-refundable fee of ₦10,000 by all interested Bidders.
6.0 Submission of Documents
The Tender documents for each lot should be Spiral bound and packaged in an envelope with evidence of payment of a non-refundable processing fee of ₦10,000 clearly marked "Procurement of Goods".
Two (2) hard copies of Technical and Two (2) hard copies of Financial bid should be submitted in two separate sealed envelopes and labelled "Technical" and "Financial" respectively. The two envelopes should be enclosed in a larger envelope and marked "Technical and Financial Bid". The envelope should be duly marked with the relevant Lot Number on the top right corner; the reverse side must bear the name and address of the prospective bidder company/firm.
All submissions are to be addressed to the Director, Procurement, Federal University of Health Sciences Otukpo and dropped in the Tenders Box at the Procurement Office, Otada Campus, not later than 12 Noon on Monday 24th August, 2026.
7.0 Opening of Tender Documents
The Tender Documents shall be opened immediately after the deadline for submission at 12 Noon on Monday 24th August, 2026 at the Lecture Hall B, Otada Campus, in the presence of Bidders or their representatives, CSOs, anti-corruption and transparency bodies, and private professional bodies, and will be covered by video recording.
8.0 Please Note
- All submissions must be in English language.
- More details regarding the scope of supplies are provided in the Bidding documents and the Bill of Quantities. Original documents on any aspect of a contractor's submission may be requested for sighting during and/or after the opening session. Documents shall be verified, and violators disqualified and reported to the investigating Authority(ies); no bidder shall bid for more than (2) Lots. All submissions shall be in line with the requirements and instructions provided above. Failure to do so will lead to outright disqualification.
- The Tender documents will be opened publicly in the presence of bidders or their representatives, CSOs, anti-corruption and transparency bodies, and private professional bodies, and will be covered by video recording.
