1.0 Preamble
In compliance with the provisions of the Public Procurement Act 2007, the Federal University of Health Sciences, Otukpo, seeks quotations from competent and qualified contractors, suppliers, and service providers to quote for the underlisted projects.

2.0 Requirements
- Evidence of company registration with the Corporate Affairs Commission (CAC) including Form C.O.2 and C.O.7.
- Evidence of current Tax Clearance Certificate valid till 31st December, 2026.
- Evidence of current Pension Compliance Certificate valid till 31st December, 2026.
- Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till December, 2026.
- Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2026.
- Evidence of Bureau of Public Procurement (BPP) Compliance Certificate expiring December 2026.
- Verifiable documentary evidence of at least three (3) similar jobs in the last five (5) years including letters of award.
- Company's audited accounts for the immediate last three (3) years: 2023, 2024 & 2025, duly acknowledged by the Federal Inland Revenue Services (FIRS).
- Reference letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when awarded.
- Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal University of Health Sciences Otukpo or the Bureau of Public Procurement is a former or present Director, Shareholder, or has any pecuniary interest in the bidder; and to confirm that all information presented in its bids are true and correct in all particulars.
- That the company is not in receivership or under any form of insolvency or bankruptcy proceeding or subject of any form of winding up petition or proceedings.
- That the company has no Director/Partner who has been convicted by any court in Nigeria or any other country for criminal offenses in relation to fraud or financial impropriety.
- That the company does not have an existing loan or financial liability with a bank, other financial institution, or third party that is classified as doubtful non-performance, bad, or whose repayment portion has been outstanding for the last four (4) months.
- Note: Affidavit submitted must be disposed as described and properly addressed to the Federal University of Health Sciences Otukpo; any affidavit that predates this advertisement will be rejected.
- Company's profile with the curriculum vitae of key staff to be deployed to the project, including copies of their academic/professional qualifications.
- Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of award and job completion certificates.
- Letter of authorization as representative from the original Equipment Manufacturer (where applicable).
- All documents for submission must be transmitted with a covering/forwarding letter on the company's/firm's letterhead paper bearing, among others, the Registration Number (RC) as issued by the CAC, contact address, telephone number (preferably GSM), and e-mail address. The letterhead must bear the names and nationalities of the Directors of the Company at the bottom of the page, and be duly signed by the authorized officer of the company/firm.
- Bidders are to ensure that all pages of the bidding documents are duly signed.
3.0 Submission of Quotations
The quotation for each lot should be spiral-bound and packaged in an envelope with evidence of payment of a non-refundable processing fee of ₦10,000 clearly marked "Procurement of Goods/Services as implied".
Two (2) hard copies of Technical and two (2) hard copies of Financial bid should be submitted in two separate sealed envelopes and labelled "Technical" and "Financial" respectively. The two envelopes should be enclosed in a larger envelope and marked "Technical and Financial Bid". The envelope should be duly marked with the relevant Lot Number on the top right corner; the reverse side must bear the name and address of the prospective bidder company/firm.
All submissions are to be addressed to the Director, Procurement, Federal University of Health Sciences Otukpo and dropped in the Tenders Box at the Procurement Office, Otada Campus, not later than Monday 26th August 2026 by 12 Noon.
Please ensure that the bid submission register is duly signed at the point of submission, as the University will not be held liable for misplaced or wrongly submitted bids.
4.0 Opening of Quotations
The quotations shall be opened the same day, Monday 26th August 2026 by 12 Noon prompt, in the presence of companies' representatives who may choose to attend.
5.0 Please Note
- Notwithstanding the submission of the technical information, the Federal University of Health Sciences Otukpo is neither committed nor obliged to include any company on any bid list.
- The University reserves the right to verify any claim by the companies. Companies shall therefore include necessary authority and guarantee for such verification; the absence of which will automatically result in the disqualification of such Contractor/Supplier.
- The University reserves the right to reject any or all bidding packages.
